Concierge Primary Care
Our care is led by licensed primary care Nurse Practitioners practicing under physician oversight. We provide comprehensive primary care services, including preventive visits, chronic condition management, diagnostics, and ongoing health monitoring.

Rather than episodic, reactive care, we focus on long-term relationships, proactive planning, and early intervention—allowing us to support patients who are generally healthy, as well as those with well-managed chronic conditions seeking more attentive care.

Care is delivered through a combination of:

• In-person visits (including mobile care when appropriate)

• Telemedicine

• Ongoing follow-up and care coordination
